Nature of Work: To provide skilled nursing care, as ordered by patient’s physician, to patient’s in their homes or place of resident.  Provide and/or arrange for, teach, and supervise nursing care of sick or injured persons of all ages.  Plan for coordinated home health care services for assigned individuals or families.  Encourage family participation in rehabilitation of patient.  Promote good health practices and educate patient and/or family about disease process, nursing care, and community resources. Works under the direction and supervision of the nurse manager.  Responsible for assisting with the assignment supervision of the Home Health Aide and L.P.N.

Education: Graduate of an Accredited School of Nursing required. Current Registered Nurse Registration with the State Board of Nursing required.

Requirements: Experience in home health care or public health nursing preferred. Evidence of independent nursing practice in delivering nursing care required. Good verbal and written communication skills required.  Good organizational and leadership skills required
