SALARY RANGE:               $19 - $26

NATURE OF WORK:

As the Clinical Care Coordinator Float, enhancement of patient access to care via schedule template management, no-showed/missed appointment follow up, staff supervision with scheduling patient care appointments and coordination of medical care services is priority. Coordination with the Director, Nurse Manager, Population Health Nurse, Information Technology Representatives and Medical Director to reach departmental goals is imperative. The Clinical Care Coordinator Float is responsible for enhancing the patient experience and effectively delivering quality customer service to patients. The Clinical Care Coordinator Float is responsible to cross-train for all Horizon Healthcare Medical Assistant and Patient Access Specialist positions including satellite’s locations on phlebotomy to fill any staffing voids as needed. The Clinical Care Coordinator Float prepares reports to be utilized by leadership for trends and decision-making. The Clinical Care Coordinator answers the multi-line phone system and takes accurate messages, assessing the urgency of the message and routing to the appropriate staff person. Performs other business office functions, such as accepting payments, handling checkout/registration, and medical record scanning/filing as needed.

Additionally, the Clinical Care Coordinator Float will assist with leading clinics in merit-based incentive payment system (MIPS) requirements to achieve incentive payments from CMS. The Clinical Care Coordinator Float ensures effective utilization of information systems at Horizon Healthcare and leads the clinics with end-user continuous NextGen learning, including medical staff, nursing, and business office functions. The Clinical Care Coordinator Float trains new users on software utilized in their job. Communication with information technology employees and managers is ongoing with progress on EHR tickets, information security issues, etc. Stays informed with NextGen notices on Critical News Flashes, for example, reviewing the problem and if clinic’s version and workflow is affected by problem, and communicating with end users accordingly. Manages and coordinates various projects through to completion - patient portal self-scheduling implementation, holding tank management for closing the loop of results and referrals, etc.

EDUCATION/QUALIFICATIONS:

High school graduate or equivalent. Certified Nursing Assistant, Certified Medical Assistant, or LPN program completion preferred.

EXPERIENCE:

Previous medical office experience and/or strong customer relations experience preferred. Strong word processing skills. Previous medical software experience and care coordination preferred.

SKILLS:

Leadership qualities are desired. Team player willing to assist in meeting organizational goals. Excellent communication skills and customer-oriented approach to others. Experience with multi-line phone system, general office equipment, computer software programs including word processing, internet usage, and email preferred.
