Fairfield Memorial Hospital’s Horizon Healthcare Clinics in Fairfield, Cisne, Grayville, and Carmi is offering school and sports physicals at no out of pocket expense to the public from June 1st to September 1st.

Fairfield Memorial Hospital’s Horizon Healthcare Clinics are located at:

  • 413 Park Street in Cisne, IL
  • 319 W. Commerce Street in Grayville, IL
  • 1103 W. Main Street in Carmi, IL

Fairfield, Cisne, and Carmi clinic locations are open Monday-Friday and accept walk-ins from 9am-4:30pm daily. Horizon Healthcare Grayville Medical Clinic is open Tuesday-Thursday from 9am-4:30pm and accepts walk-ins. You do not have to be a patient of Horizon Healthcare to take advantage of this special offer.

We will also be providing school and sports physicals at school registrations throughout Wayne, White, and Edwards Counties.

Clay City - Wednesday, July 26th - 10a-1p

Wayne City - Tuesday, August 1st - 12p-5p

North Wayne - Thursday, August 3rd - 2-6p (Cisne Middle School for all North Wayne Schools)
