In coordination with the State of Illinois, Fairfield Memorial Hospital and Horizon Healthcare will be hosting special Pfizer COVID vaccinations to patients from the ages of 12 to 17, as well as, any adult that lives in the State of Illinois.

The COVID vaccinations will be administered by appointment and walk-ins will be welcomed. To schedule an appointment, please call 618-842-2082. Below is a list of dates, times, and locations that these vaccinations will be given:

Monday 6/28/2021, Horizon Healthcare in Fairfield from 1pm-5pm

Tuesday 6/29/2021, Horizon Healthcare in Carmi from 2pm-6pm

Tuesday 7/6/2021, Horizon Healthcare in Fairfield from 2pm-6pm

Wednesday 7/7/2021, Horizon Healthcare in Carmi 1pm-5pm

Monday 7/12/2021, Horizon Healthcare in Fairfield 1pm-5pm

Wednesday 7/14, Horizon Healthcare in Carmi 1pm-5pm

You do not have to be a patient of Horizon Healthcare to receive this vaccination. Patients 12 through 17 must be accompanied by a parent or a legal guardian. Every parent/legal guardian will be asked to bring ID and proof of insurance, if applicable.  Patients over 18 years of age and older must be able to provide a valid ID showing residency in the State of Illinois, as well as, proof of insurance, if applicable. The Pfizer COVID-19 vaccine is a two-part vaccination series and your appointment for the second vaccination will be scheduled at the time you receive your first vaccination.

Masks will be required as we are a healthcare organization and guidelines for wearing masks in healthcare facilities have not changed with the State of Illinois moving into Phase 5. All consents for vaccinations MUST be signed by legal parent or guardian for anyone under the age of 18.

It is highly recommended that if you have an epi-pen to bring it with you to your vaccination appointment. A physician, as well as, other trained FMH staff will be on duty in case of a reaction as patient safety is our highest priority. You should also wear appropriate clothing for receiving the vaccination as there will not be a way to provide for your privacy. The vaccination is given in the upper part of your arm so long sleeve shirts are not recommended.  All recipients of the vaccination will remain under observation in the MAC education room by a physician and an RN for a minimum of 15 minutes.  Any patient who complains of possible adverse reaction symptoms will remain under observation until they are assessed and released by the physician on site.
